SS-31

Also known as Elamipretide

SS-31, better known as elamipretide and also called MTP-131, is a four-amino-acid Szeto-Schiller peptide with the sequence D-Arg-Dmt-Lys-Phe-NH2. It homes in on the inner membrane of mitochondria. In September 2025 the FDA granted accelerated approval to elamipretide hydrochloride, sold as FORZINITY, for a single rare disease.

What researchers study it for

  • Muscle strength in people with Barth syndrome
  • Muscle disease caused by mitochondrial defects
  • Heart failure of the reduced-ejection-fraction type
  • Macular degeneration linked to aging
  • Cardiolipin and how mitochondria make energy

How it is thought to work

The peptide attaches to cardiolipin, a fat molecule that sits in the inner mitochondrial membrane, using both charge and water-repelling contacts. In bench experiments, that attachment keeps cardiolipin from turning cytochrome c into a peroxidase enzyme while letting cytochrome c keep ferrying electrons; researchers also saw cristae folds and oxidative phosphorylation stay intact. On the FDA label the drug is classed as a mitochondrial cardiolipin binder.

Where the research stands

Accelerated FDA approval for Barth syndrome. The approval for FORZINITY (NDA 215244) came on September 19, 2025, with a stated indication of improving muscle strength in Barth syndrome for patients of at least 30 kg body weight. It relied on an intermediate endpoint, so staying approved depends on confirmatory studies. Every other use is still experimental.
